Attire to Hire Goodwill Program
Hi,
Here is the form and instructions.
I have attached a blank Goodwill Voucher request form to be filled out completely except for the last three lines (in red), it must be returned as a Microsoft word document. Vouchers are to be filled out one per person (children need their own voucher). I will let you know when the voucher has been processed, I will include the expiration date and the store the voucher was sent to in the email. Please let me know if you have any questions while completing the voucher request form.
